Your Privacy Matters
This privacy policy explains how we use any personal information we collect about you. It is made available to data subjects prior to Florida Rehability processing their personal data.
Information We Collect: The only information we collect about you is information you provide (phone/email address) when you request additional information and through out the onboarding process (your HR files as required for you provide home health services).
How We Use Your Data: The personal information that you provide to us is used to perform our contractual obligations with you, as well as to communicate with you as per your consent based on legitimate needs including patient care, updating HR files and recruitment efforts.
Florida Rehability will never sell, rent, or disclose your personal information to third-party entities.
Home Health Agencies who contract with Florida Rehability will have access to your information as necessary to remain compliant with government regulations and to ensure HR files are complete and up to date.
It is possible that we will need to disclose information about you when required by law, subpoena, or other legal process, or if we have a good faith belief that disclosure is reasonably necessary to:
Investigate, prevent, or take action regarding suspected or actual illegal activities or to assist government enforcement agencies,
Enforce our agreements with you,
Investigate and defend ourselves against any third-party claims or allegations,
We will attempt to notify you about legal demands for your personal data when appropriate in our judgment, unless prohibited by law or court order or when the request is an emergency. We may dispute such demands when we believe, in our discretion, that the requests are overbroad, vague or lack proper authority, but we do not promise to challenge any demand.
Data Retention: Florida Rehability retains your personal information (HR files) for as long as necessary to comply with our legal or contractual obligations.
Your Rights as Data Subject:
At any point while we are in possession of or processing your personal data, you, the data subject, have the following rights:
Right of access – you have the right to request a copy of the information that we hold about you.
Right of rectification – you have a right to correct data that we hold about you that is inaccurate or incomplete.
Right to be forgotten – in certain circumstances you can ask for the data we hold about you to be erased from our records.
Right to restriction of processing – where certain conditions apply to have a right to restrict the processing.
Right of portability – you have the right to have the data we hold about you transferred to another organization.
Your Consent: By consenting to this privacy policy you are giving us permission to process your personal data specifically for the purposes identified. Consent is required for Florida Rehability to process personal data for the purposes stated above, but it must be explicitly given. When we ask you for sensitive personal data we will always tell you why, and how the information will be used.
